[ARCHIVED] NTSB Releases Final Investigative Report on Fatal 2017 Explosion

FREDERICK, COLO. – Today the National Transportation Safety Board (NTSB) released its final report into the explosion that occurred in the Town of Firestone on April 17, 2017.


The home explosion, which resulted in the deaths of Mark Martinez and Joey Irwin, was confirmed by fire investigators in early May 2017 to be due to un-odorized gas from a nearby well traveling through an uncapped, abandoned well line. Before NTSB entered the investigation, Frederick-Firestone Fire District investigators worked closely with The Colorado Bureau of Investigation (CBI), Firestone Police Department and the Colorado Oil and Gas Commission (COGCC) and released their findings in a press conference held at the Fire District’s Administration Building in Frederick, Colorado on May 2, 2017.


The Frederick-Firestone Fire Protection District and the NTSB have worked closely throughout this lengthy and ongoing investigation.
